台灣血栓暨止血學會101年度聯合年會，將於101 年5 月19 日(星期六)假台大醫學院102講堂舉行，敬邀會員踴躍參加。
本學會101年度第一屆第二次會員大會謹訂於101年5 月19日16:00~16:30 於同場地(102講堂)召開，敬請準時出席。
若不克出席本次會員大會者，請填妥委託書並簽名，委由其他本學會會員代為出席。

注意事項:
一、為使報到順利進行，活動當天請依 會員編號 報到。
會員編號查詢方式：至本學會網站上查詢 會員編號查詢系統 ；或來電洽詢 02-23578595吳伊婷。
二、 有受委託之會員，請持委託書於報到處報到。
三、 報到時間：101年5月19日 08:30-08:50
四、101年5月19日(六)18:00將舉辦年會晚宴，敬邀會員蒞臨參加。欲參加之會員請來電或來信報名，以便統計人數。
五、發表者請事先準備PowerPoint 簡報檔案，並於發表前，至發表場地測試。或事先將報告電子檔e-mail至tsth.tw@gmail.com，以利會議順利進行。
六、會員及發表者中午可使用本學會所發放的200元餐券至台大醫院東址(新大樓)地下一樓餐廳用餐。(餐券限本人親自領取，不得代領)
七、教育積分：本會已申請到中華民國血液病學會學分-丙類3.5分，台灣臨床病理檢驗醫學會學分-7分、台灣內科醫學會-10分，台灣兒科醫學會-2分、同時也已向中華民國心臟學會提出教育積分申請。
